Physical Therapy

The role of physical therapy is critical in restoring functionality, aiding injury recovery, and alleviating pain. Our dedicated team specializes in therapeutic approaches tailored to the needs of seniors, addressing concerns related to flexibility, posture, balance, and coordination.

Our holistic approach to care often involves pairing physical therapy with occupational therapy, speech therapy, and additional skilled nursing services. At Heritage Health, we believe in a team-based approach to care, offering comprehensive services to equip seniors with the strength, mobility, and confidence necessary to return home.

The duration of a short-term rehab stay varies among individuals and depends on the severity of the surgery, procedure, illness, or medical condition. If rehabilitation needs become extensive, we offer longer-term rehab programs and long-term care options. Contact Heritage Health to explore our wide range of senior therapy options or book your stay today.

E-Stim

This innovative approach serves as an excellent alternative for those who find traditional physical therapy too demanding, ensuring a supportive and beneficial recovery journey. E-Stim, through electrical pulses, counteracts muscle decline in seniors, addressing the susceptibility of skeletal muscle to aging effects. This treatment, designed to strengthen muscles with less pain than traditional physical therapy, is effective for seniors experiencing pain or limited mobility due to injuries or illnesses. Administered by physical therapists, E-Stim is a passive activity, allowing residents to relax while targeting specific areas of pain or limited mobility. Successful in treating various conditions like low back pain, post-surgical pain, muscle weakness, and tendonitis, E-Stim is part of our commitment to staying at the forefront of healing technologies, ensuring the best care for residents' health and comfort.

Occupational Therapy

Our unparalleled occupational therapy services are offered to seniors facing motor function loss due to medical issues. Tailored to individual needs and goals, our occupational therapy programs aim to enhance daily life skills, promoting independence and a higher quality of life. Working collaboratively with the physical therapy team, occupational therapists guide patients through tasks like getting ready for the day and household chores, fostering confidence and techniques for safe navigation. Our Living Skills Retraining program focuses on home activities, ensuring seniors can manage kitchen duties, laundry, personal hygiene, and leisure activities. Before returning home, therapists conduct a home assessment to ensure safety, identify hazards, and assist with necessary equipment setup, aiming for a return to everyday life with confidence and security.

Short Term Rehab

At the heart of our mission lies a commitment to providing outstanding, patient-centered care for seniors as they heal in a nurturing and comfortable environment. We strive to facilitate successful recoveries that enable our residents to return home safely, fully equipped and confident to resume their daily activities. Our goal is not just recovery, but empowering each individual in our care to regain their strength, independence, and joy of living.

Short-term rehabilitation, also known as rehab therapy, serves as a vital bridge between the hospital and home. It's a non-hospital setting that delivers medical care along with therapeutic services to aid patients in their recovery journey from an injury, illness, or surgery. The duration of a patient's stay can vary from a few days to several weeks, and occasionally up to 100 days, depending on the individual's recovery needs.

Compared to acute care, short-term rehabilitation is more intensive, featuring longer treatment sessions, targeted exercise training, and specific education to prepare patients for their return home. This approach ensures our patients are well-equipped to reintegrate into their daily lives post-rehabilitation.

Speech Therapy

Providing effective speech therapy programs and addressing challenges related to speech and swallowing stemming from illness or age helps improve our residents’ quality of life. Our skilled speech therapists focus on supporting seniors with conditions such as cardiac episodes, nervous system disorders, and esophageal problems, including dysphagia. This swallowing disorder can lead to difficulties with liquids, foods, or saliva, impacting nutritional intake and resulting in weight loss. Therapists offer personalized programs, incorporating muscle exercises, head postures, and new dietary approaches for non-invasive muscle strengthening and swallowing rehabilitation.

Outpatient Therapy

These outpatient therapy services cater to those recovering from illnesses, surgeries, or hospital stays, including accidents, injuries, or procedures like joint replacements. With a focus on creating individualized treatment plans, outpatient therapy allows seniors to recover at home while maintaining a typical daily routine and accessing exceptional therapeutic care. Our experienced team in multiple therapy disciplines ensures a tailored approach to specific recovery goals, bringing decades of excellent senior care beyond campus grounds to homes.

IV Care

The skilled nursing staff at Heritage Health Therapy and Senior Care offers specialized care for seniors needing intravenous therapy (IV) administration. Aging skin and potential complications underscore the importance of professional skills in delivering fluids directly into a vein. IVs are widely used for various health concerns, and our professionally trained nursing team can administer them to provide medication, fluid replacement, hydration, and correct electrolyte imbalances swiftly.

This method proves effective in delivering substances directly into the body, benefiting seniors in scenarios like addressing low potassium levels detected during routine check-ups. Whether for temporary medication or managing permanent IV therapy, our medical staff collaborates with residents' physicians to ensure proper care, considering factors like dosage, frequency, and administration. Recognizing the significance of these treatments for seniors, Heritage Health Therapy and Senior Care offers compassionate and skilled administration of IV therapy.

Tracheostomy Care

Due to various health issues, a senior may come to Heritage Health Therapy and Senior Care with a breathing tube, placed in the trachea as a result of a tracheotomy. Our skilled nursing team ensures optimal care as seniors recover from throat obstruction, lung issues, or other similar medical issues. Following the procedure, residents may have limited communication, and our compassionate staff supports them emotionally while delivering necessary medical care. Understanding the challenges posed by communication barriers, our compassionate and seasoned team is dedicated to providing outstanding senior care. We prioritize health, comfort, and all-encompassing support, ensuring a nurturing environment for our seniors.

Respite Care

Respite care at Heritage Health Therapy and Senior Care offers a short-term stay program for seniors, providing a much-needed break for caregivers to recharge. We provide around-the-clock care by skilled and compassionate staff, ensuring continuous support for seniors during their stay. This option is distinct from daycare services, offering comprehensive care throughout the duration of the stay, not just during the day. Respite care becomes valuable when caregivers need to leave home for business trips or personal reasons, allowing their loved ones to stay on our campus with access to services such as:

  • -medication management
  • -personal assistance
  • -24-hour nursing care
  • -socialization and activities
  • -mealtime assistance
  • -quality nutrition options
  • -laundry
  • -housekeeping services

For those considering a permanent home for their loved ones, respite care serves as an opportunity to familiarize seniors with the nursing home environment and team during a short stay. Contact us today to inquire about respite care options, knowing that our caring and compassionate team will treat your family as part of ours.

Wound Care

Wound care is a crucial concern for seniors, given the aging process's impact on skin fragility and healing abilities. Surgical incisions, pressure, and accidents can lead to problematic wounds, especially when compounded by conditions like diabetes and venous hypertension. Heritage Health Therapy and Senior Care addresses wound care comprehensively, emphasizing both treatment and prevention. The skilled nursing team, working in collaboration with wound physicians, primary care physicians, and other specialists, creates personalized care plans for various wounds, including pressure ulcers, venous ulcers, arterial wounds, diabetic neuropathic wounds, and post-surgical wounds. The dedicated focus on wound care at Heritage aims for faster healing, improved infection management, and reduced hospital stays, enhancing clinical outcomes which is appreciated by seniors and their families.

Medicare

We are proud to offer Medicare-Certified skilled nursing care, ensuring that the level of care provided meets or exceeds all standards established by the Medicare Federal Health Insurance program. Seniors meeting criteria can benefit from reimbursements, fully or partially covering their short-term skilled care needs through Medicare. Medicare, a federal health insurance program for individuals over 65, includes different parts:

Part A is hospital insurance, which covers senior hospital stays, care in a skilled nursing home, hospice care, and some home health care. This is the type of Medicare that covers the seniors’ inpatient hospital stay, and short-term skilled nursing stays with certain restrictions. Medicare covers skilled nursing care if a senior spends at least three days in a hospital.

Part B is medical insurance and includes certain doctors’ services, outpatient care, medical supplies, and preventive services.

Part C is a combination of each of the previously mentioned parts packaged together for one cost.

Part D is prescription drug coverage.

Typically, if a senior has spent at least three days in the hospital and requires skilled nursing care, Medicare Part A covers a short-term stay. After a certain period, extended services may be covered with supplementary insurance like Blue Cross Blue Shield. The Heritage Health team, with decades of experience working with the Medicare program, helps seniors navigate their benefits and offers detailed information about coverage before and during their stay.

Dietary Needs

Our dietary professionals play a crucial role in ensuring the nutritional needs of residents are met with flavorful and fresh meals. We provide three healthy and delicious meals daily, accommodating various dietary needs such as low-sodium, diabetic-conscious, or pureed options. Caloric needs are calculated and monitored for each resident, and snacks and drinks are provided. Our culinary team develops menus catering to dietary restrictions and individual needs. As part of our commitment to caring for the whole person, we consider seniors' medical history and dietary preferences, collaborating with certified dietitians to align meal plans with health practices. For residents with conditions like diabetes, our staff monitors sugar intake closely. Mealtime serves as an opportunity for socializing, and families are welcome to join for lunch or dinner.
