What is a skilled nursing facility?

Our skilled nursing facility is a healthcare setting that provides specialized and comprehensive medical care, including 24/7 nursing supervision, rehabilitation services, and assistance with daily activities such as medication management or showering assistance. The staff at Heritage Health Therapy and Senior Care cares for individuals who require more intensive medical attention and rehabilitation than what can be provided at home or in assisted living communities.

What is the cost?

With a variety of payment options including Medicare benefits, Medicaid benefits, and supplemental insurance policies, pricing can vary. We’re happy to expand on these benefits and payment options during your tour or as you make plans to move in.

Can couples stay together?

Of course! When couples move into our facility they will each be assigned a connected room and bathroom. They are welcome to set up one room as a living space and the other as their bedroom.

What social activities are available for residents?

Our social calendars are always packed with parties at Heritage Health Therapy and Senior Care – from holidays to carnivals to movie nights our residents always have an event to attend where they can have fun and socialize with their neighbors. The close knit community of Hoopeston loves to support our seniors with frequent visits from students and volunteer groups.

Are vacations allowed?

Our residents are free to take any trip for any length they would like as long as they have been properly signed out.

Can personal items be brought for the resident's room?

Yes, residents can bring in their personal items such as photos and throw blankets to make their room feel more like home. Our helpful maintenance team can help hang any pictures as well!

What should I bring?

What happens in case of a medical emergency or hospitalization?

We are located only 3 short miles from Carle Hospital and will take residents there along with communicating to their families.

What is the staff-to-resident ratio?

We have a staff-to-resident ratio of 1 to 8 allowing our highly skilled and dedicated staff to provide personalized care to our residents. With a largely local team we experience very little staff turn over so you can expect to see the same friendly faces who get to know what juice your loved one wants with breakfast and how they want their bed made.

Are the rooms private?

Yes, all of our rooms are private! Residents will share a Jack and Jill style bathroom and enjoy the privacy and serenity of their own space.

What day-to-day necessities are offered?

From an onsite beautician to a beautiful courtyard and grounds with raised gardening beds, Heritage Health Therapy and Senior Care has tons of wonderful amenities , visit our Our Community page to learn more.

Are animals allowed?

While animals are not allowed to live with us, we never turn down a visit from our residents’ furry friends.

How do I visit?

We love nothing more than welcoming our resident’s guests who are free to visit anytime. Be sure to come in through the main entrance to sign in and then you are free to enjoy our beautiful community with your loved one.

What types of medical care and assistance will be provided?

Our compassionate medical professionals will provide skilled nursing needs ordered by your physician such as IV therapy, total parenteral nutrition (TPN), medication management, catheter care, wound care, tracheostomy care, and facilitate hospice care if needed. Our contracted services with Select Therapy offer programs including physical, occupational, outpatient, and speech therapies.
