Amber’s longevity with the Hoopeston Nursing Home and subsequently Heritage Health are a huge benefit to our facility. Amber is charged with overseeing the nursing staff and maintaining high standards of quality care for all our residents.

Assistant Director of Nursing/Admissions/Wound Nurse
As the ADON, Trisha is Amber’s second in command. She is also tasked with auditing and training nursing staff. Her other duties include arranging admission to our facility and helping families through that process, as well as working with hospital discharge planners.

MDS Nurse
MDS (Minimum Data Set) is the comprehensive, standardized assessment of each resident's functional capabilities and health needs. Jamie is tasked with completing and verifying each of the assessments we use to make sure that each resident is given individualized, quality care. Jamie also arranges the quarterly care conferences with families.

The infection preventionist is responsible for identifying, investigating, monitoring, and reporting healthcare-associated infections and communicable diseases, looking for patterns of infection within the facility, observing practices, and providing education to the facility. Meagan is also the one who helps coordinate discharge planning between the family and doctor to ensure a safe transition from a skilled nursing stay back to home.

Resident choice meals and theme meals are her favorite so don’t be surprised if you see her donning a cowboy hat or playing Italian music when the theme meals are being served.
